Support to take part in community life — activities, groups, errands and everyday places — at a pace that feels comfortable and with as much independence as possible.

What community inclusion support can include

  • Planning an outing together, then going along as a familiar, steady presence
  • Getting to recreation centres, libraries, classes, faith communities, markets or volunteer roles
  • Practising the practical steps of an activity — bus routes, buying a ticket, ordering at a counter
  • Support with social connection: joining a group, keeping in touch with people, building a weekly rhythm
  • Working towards personal goals in the community, and reviewing how they are going

Who it may help

People who would like to be out in their community more but find the planning, travel or unfamiliarity difficult to manage alone. That may be an adult building confidence after a change in circumstances, someone who has become isolated at home, or a person working on independence goals with their family and support network.

Our scope and how we work

Community inclusion support from Evercare is non-medical. Support workers do not provide nursing, clinical treatment, therapy or medical assessment. We start with a conversation about what a good week looks like, agree what support is actually wanted, and keep the same worker involved wherever scheduling allows so routines stay familiar.

Inclusion support is often combined with daily living support or companion support, so the week holds together rather than sitting in separate pieces.
