Regular, goal-directed support for people living in their own place in the community — practical, non-medical and built around the outcomes that matter to them.
What supported living and outreach can include
- Scheduled outreach visits at home, with agreed goals for each visit
- Support to keep a tenancy running smoothly: household routines, shopping, organizing bills and paperwork
- Building skills step by step, with the aim of needing less support over time where that is realistic
- Coordination in practical terms — reminders, planning ahead, preparing for appointments
- Working alongside the person's own network, at the level of involvement they choose
Who it may help
Adults living independently or semi-independently who want reliable support without someone taking over. Outreach suits people who need a set number of hours each week rather than continuous presence, and who are working towards specific goals at home and in their community.
Our scope and how we work
Evercare provides non-medical community support only. We do not provide nursing, clinical or health services, and we do not make funding or eligibility decisions of any kind. If you are exploring how support might be arranged or funded, speak with the relevant organization directly — we will be honest about what we do and do not know.
