Not everyone fits neatly into one service. Personalized support combines the pieces that are genuinely useful into a single, coherent plan.
How a personalized plan is put together
- We start with the person's own description of a good week, and what currently gets in the way.
- We identify which supports would actually help — household, personal, community, or a mix.
- We agree hours, days and who does what, including the tasks the person prefers to keep doing themselves.
- We review the plan as circumstances change, so it stays relevant rather than fixed.

Every combination stays within non-medical community support. Evercare does not provide nursing, clinical treatment, therapy or medical assessments, and we do not make promises about funding, eligibility or approval by any other organization.
